About The Position

The role of the Homecare Coordinator reports to the PACE Center Manager. This position is responsible for coordinating clerical services as it pertains to the homecare PACE program services. This position will tracking approved home care hours within the designated home care database for billing purposes, serving as a liaison between PACE home care and the home care agencies, and ordering home care incontinence supplies.

Requirements

  • High school diploma or GED
  • CPR, required
  • Minimum of two years’ experience providing in-home care, preferred.
  • Previous experience with the elderly population, preferred.

Nice To Haves

  • Certified Nursing Aide or LPN, preferred.

Responsibilities

  • Assist with coordinating necessary supplies with participants
  • Serve as point of contact between participants and agencies for scheduling in-home care services
  • Review in-home care service records to ensure insurance billing requirements are met
  • Partner with leadership team to manage and prioritize utilization of home care needs
  • Communicate with PACE team to prioritize homecare needs
  • Establish and maintain effective working relationships with program staff, agencies, and participants
  • Remain knowledgeable and current regarding PACE program regulatory requirements and program initiatives
