As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Schedule: Monday-Friday 8:00am-4:30pm with occasional evening and weekend community events Position Summary: The PACE Outreach Coordinator is responsible for developing, implementing, and monitoring outreach strategies to increase awareness of and referrals to the PACE program. This role focuses on expanding access to PACE services through education, relationship-building, and community engagement with senior housing providers, healthcare organizations, referral sources, and community partners within the service area. The Outreach Coordinator works closely with the PACE Participant Enrollment Specialist to support a smooth and timely referral and intake process, ensuring effective handoff from referral source to the PACE team. This role also collaborates with internal clinical service line leaders to identify individuals within Bluegrass Care Navigators (BCN) who may be eligible for PACE services and assist in facilitating appropriate transitions into the program.

Requirements

  • Bachelor’s degree or higher, preferably in healthcare, human services, or a related field; or equivalent combination of education and experience.
  • Minimum of two years’ experience in direct sales or outreach; healthcare experience preferred.
  • At least one year of experience working with frail or elderly populations.
  • Food handling permit required (to be completed upon hire).
  • Excellent communication skills, including face-to-face, telephone, and written communication; strong computer literacy.
  • Strong attention to detail and organizational skills.
  • Ability to interact professionally and successfully with participants, families, referral sources, and other agencies, presenting a positive image of the organization.
  • Ability to exercise judgment and discretion when handling confidential information.
  • Excellent public speaking and presentation skills.
  • Ability to manage multiple priorities effectively and handle the emotional demands of the workload.
  • Self-directed, able to work independently while exercising a high degree of discretion and judgment.
  • Physical ability to access homes, hospitals, nursing homes, and assisted living facilities, including navigating stairs, narrow spaces, and varying environmental conditions.
  • Ability and willingness to travel within the community as required.
  • Availability to work evening or weekend events as needed.
  • Must be a licensed driver
