About The Position

The Home Infusion Coordinator at Polaris Pharmacy Services is responsible for coordinating and managing the delivery of home infusion therapy services to patients. This role ensures that patients receive high-quality, timely, and efficient care while maintaining compliance with all regulatory and company standards. The coordinator acts as a liaison between patients, healthcare providers, and the pharmacy to facilitate seamless service delivery.

Requirements

Strong knowledge of home infusion therapies, including medication administration and patient monitoring.
Excellent organizational skills with the ability to manage multiple tasks and prioritize effectively.
Proficient in using healthcare management software and electronic medical records.

High School diploma or equivalent required
Minimum of 3 years of experience in home infusion therapy or a related healthcare setting.
Proven experience in coordinating patient care and managing infusion services.
Experience working with interdisciplinary teams to deliver comprehensive patient care.

Responsibilities

Coordinate the initiation and continuation of home infusion therapy services for patients.
Serve as the primary point of contact for patients, healthcare providers, and internal teams regarding home infusion services.
Ensure accurate and timely communication of patient information and therapy requirements to all relevant parties.
Schedule and oversee the delivery of medications and supplies to patients' homes, ensuring timely and accurate fulfillment.
Monitor patient therapy progress and address any issues or concerns in collaboration with healthcare providers.
Maintain comprehensive and accurate patient records, ensuring compliance with all regulatory and company standards.
Collaborate with pharmacists, nurses, and other healthcare professionals to optimize patient care and outcomes.
Provide education and support to patients and their families regarding home infusion therapy and related processes.
Participate in quality improvement initiatives to enhance service delivery and patient satisfaction.
Stay informed of industry trends and best practices to continuously improve the home infusion program.
