Requirements

  • High School Diploma or GED - some college preferred
  • Able to work onsite Monday - Friday, 8:00 am to 5:00 pm
  • May be required to work later to ensure completion of work
  • Understands HIPAA compliance
  • Minimum 2 years of experience in service coordination in healthcare, staffing, or related industry (preferrably Home Health)
  • Proven organizational skills and detail orientation
  • Ability to multi-task, excellent communication skills
  • Daily demonstration of a customer service mindset

Responsibilities

  • Provides initial Customer Service to patients, family members, referral sources, and fellow healthcare providers, to ensure a complete referral is received and entered in the E.H.R. system.
  • Monitors and uses Forcura, Outlook email, (CarePort/Wellsky, Aidin, Aida, Ensocare, NaviHealth, EPIC EMR portals, and any additional assigned portals), timely and responds within 60 minutes to referral source/BDRs
  • Communicates with patient/client and family regarding arrangements for the initiation of care/services
  • Receives and enters complete referral to include all the information listed in process
  • Completes verification/eligibility and initial auth and enters information during the referral entry process
  • Provide a centralized intake and client service function to gather pertinent patient/client, clinical, and financial information to optimize sales, customer responsiveness and satisfaction, and quality care
