About The Position

A new Home Health agency is launching in Waco, TX, and we are seeking a Clinical Director who can lead the clinical program from the ground up. This position plays a crucial role in shaping patient care standards, developing strong field teams, and ensuring regulatory excellence from day one. This is a leadership opportunity for a Home Health RN who brings sharp clinical judgment, strong operational understanding, and a passion for helping patients remain safe, supported, and independent in their homes.

Requirements

  • Bachelors Degree in Nursing (BSN) or related field.
  • Active RN license in the state of Texas.
  • Minimum 2 years of clinical Home Health experience , including OASIS familiarity.
  • Strong understanding of Medicare Home Health CoPs (Conditions of Participation).
  • Excellent communication, assessment, and leadership skills.
  • Ability to lift up to 50 lbs and assist patients as needed.
  • Must be able to bend, walk, climb stairs, and navigate variable home environments.
  • Ability to respond quickly and calmly during urgent situations.
  • Consistent attendance and reliability are essential.

Nice To Haves

  • Previous Home Health management or supervisory experience.
  • Comfort leading multidisciplinary teams and implementing new processes.

Responsibilities

  • Clinical Oversight & Quality Assurance Oversee daily clinical operations, field staff performance, and patient care delivery.
  • E nsure compliance with Medicare/Medicaid Home Health regulations, OASIS accuracy, and agency policies.
  • Review, approve, and monitor plans of care to ensure individualized, safe, and goal-driven services.
  • Conduct chart audits and quality checks to maintain excellence in documentation, outcomes, and regulatory readiness.
  • Team Leadership & Development Manage and mentor RNs, LVNs, CNAs, and therapy staff through ongoing coaching, education, and leadership.
  • Recruit, hire, orient, and retain high-quality clinical talent to maintain strong caseload coverage.
  • Provide clear expectations, constructive feedback, and ongoing training to support professional growth.
  • Set work schedules, coordinate coverage, and ensure field staff meet productivity and quality standards.
  • Operational Excellence & Coordination Partner closely with the Administrator to support efficient operations and workflow.
  • Ensure timely documentation and escalation of any patient condition changes.
  • Monitor utilization, visit frequency, and care pathways to improve outcomes and cost effectiveness.
  • Maintain readiness for state surveys, accreditation reviews, and payer audits.
  • Family & Community Communication S erve as a clinical resource for families, answering questions and resolving concerns with clarity and compassion.
  • Build strong relationships with referral sources, physicians, and community partners.
