About The Position

Choices Home Health is seeking a relationship-driven sales professional to serve as a Business Development Liaison. This high-visibility role involves owning a territory, building strong referral pipelines, and positioning Choices Home Health as the preferred provider in the community. The successful candidate will be instrumental in driving growth and improving patient lives by turning connections into meaningful patient care opportunities.

Requirements

  • Proven ability to build and maintain referral relationships
  • Strong understanding of the healthcare landscape and patient flow
  • Excellent communication, presentation, and follow-up skills
  • Self-motivated, goal-oriented, and comfortable working independently
  • Valid driver’s license and auto insurance

Nice To Haves

  • 2+ years of home health or hospice sales experience
  • Experience using CRM systems (HomeCare HomeBase is a plus)

Responsibilities

  • Build and grow referral relationships with physicians, hospitals, case managers, and community partners
  • Represent Choices Home Health with confidence, clearly communicating our value and care model
  • Identify and develop new business opportunities within your territory
  • Collaborate with clinical and leadership teams to align services with market needs
  • Provide insight into community trends, helping shape new programs and service lines
  • Lead outreach and public relations efforts to strengthen brand presence
  • Ensure compliance with all healthcare marketing regulations
  • Drive admissions growth and consistently meet (or exceed) monthly targets
