About The Position

At Choices Home Health, we are seeking a motivated Sales Liaison who excels in field-based work, possesses strong local market knowledge, and is prepared to manage and expand their assigned territory to drive significant growth. This role involves cultivating and maintaining high-value referral relationships with various healthcare entities such as physicians, hospitals, case managers, skilled nursing facilities (SNFs), and community partners to consistently generate new business. The Sales Liaison will strategically position Choices Home Health as the preferred provider through direct, in-person outreach, identify new accounts, initiate contact, and secure referral opportunities. Essential to this position is collaboration with clinical teams to ensure seamless patient transitions and the delivery of exceptional service. The individual will also be responsible for monitoring market trends and competitor activities, implementing targeted outreach and marketing strategies to boost admissions, and consistently achieving or surpassing sales and admission targets.

Requirements

  • Minimum of 2 years home health or hospice sales experience
  • Valid Driver’s License
  • Valid Auto Insurance
