About The Position

Midland Care Connection Midland Care Connection is a not-for-profit, community-based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. PACE Program The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity. Home Health Aide Provides hands-on personal care and daily living support to help older adults remain safe, comfortable, and independent in their homes and community. They assist with activities of daily living such as bathing, dressing, grooming, toileting, mobility, and meal support, while also offering companionship and emotional support. HHAs monitor basic health indicators, observe and report changes in a participant’s condition, and follow individualized care plans developed by the interdisciplinary PACE team. By supporting both personal care needs and overall well-being, HHAs play a vital role in helping PACE participants age with dignity and avoid unnecessary institutional care.

Requirements

  • Current Home Health Aide (HHA) certification, as required by state regulations
  • High school diploma or GED
  • Ability to assist with activities of daily living such as bathing, dressing, toileting, transfers, and mobility
  • Basic knowledge of infection control, safety practices, and personal care techniques
  • Ability to observe, document, and report changes in participant condition to clinical staff
  • Reliable transportation and valid driver’s license (if travel between homes or sites is required)
  • Ability to work effectively as part of an interdisciplinary care team
  • Physical ability to perform job duties, including lifting, bending, and standing for extended periods

Nice To Haves

  • Previous experience working with elderly, frail, or disabled adults
  • Experience in a PACE program, home health, long-term care, or assisted living setting
  • CPR and First Aid certification
  • Strong communication and interpersonal skills
  • Compassionate, patient, and dependable approach to caregiving
  • Experience supporting individuals with chronic conditions, dementia, or mobility limitations
