About The Position

The Home Coordinator is responsible for the daily supervision and operation of assigned residential group homes serving individuals with intellectual disabilities. This role involves direct oversight of program operations, team member supervision including performance management and scheduling, and ensuring compliance with ODP staff regulations. The coordinator is also responsible for medication management, electronic health record documentation, medical training for staff, and coordinating medical appointments for individuals. Effective communication within the team and with external supports is crucial. The position also involves participation in hiring processes, employee relations, disciplinary actions, and assisting with annual licensing preparations. This is a full-time, non-exempt position requiring flexibility based on program needs.

Requirements

  • Associate’s Degree in human services, social work, psychology, or related field (OR) a High School Diploma or equivalent and two (2) years of experience.
  • Demonstrates interest in the development of others and positively impacts the lives of patients/families, peers, and members of the Hope team through mentoring, education, and knowledge sharing.
  • Valid driver’s license.

Responsibilities

  • Direct and oversee daily program operations within assigned residential group homes through the supervision of team members, including performance evaluations and management, scheduling, and ODP staff compliance.
  • Visit at a minimum, one group home per day and will visit all assigned residential group homes within their responsibility at least once a week.
  • Participate in interviews, selection, and the onboarding process of new hires under the supervision of the Supports & Services Manager (SSM).
  • Ensure services are provided in accordance with agency policies and procedures.
  • Direct and oversee medication management, electronic health record documentation and monitoring, medical training for staff regarding individual’s diagnoses and medical conditions, and staff support for health/medical related questions.
  • Coordinate medical appointments for individuals including transportation and follow-up.
  • Facilitate effective communication within the team, including external individual supports.
  • Participate in employee relations activities with the team and engage in disciplinary actions, as needed, with assistance from Human Resources.
  • Assist with annual licensing preparation and plans of corrections as needed.
  • Partner with leadership team to manage the communications for staff and leadership including annual enrollment guides, new hire guides, etc.
  • Coach and mentor direct reports; provide training, oversee performance, and aid with career progression.
  • Perform other related duties and assignments as directed by the Support and Services Manager.
  • Serve as the backup for the positions supervised.
  • Perform basic math functions, write, read, and comprehend what is read.
  • Utilize expressive and receptive language skills.
  • Transfer and lift individuals with or without assistance and to occasionally push/pull.
  • Perform twisting, bending, and other physical movements at varying degrees.
