About The Position

The Home Coordinator is responsible for the daily supervision and operation of assigned residential group homes serving individuals with intellectual disabilities. This role involves direct oversight of daily program operations, including team member supervision, performance evaluations and management, scheduling, and ODP staff compliance. The coordinator is required to visit at least one group home per day and all assigned homes weekly. They participate in the interview, selection, and onboarding process for new hires, and ensure services adhere to agency policies. Key responsibilities also include managing medication, electronic health records, providing medical training for staff, and coordinating medical appointments. The Home Coordinator facilitates team communication, engages in employee relations and disciplinary actions with HR assistance, and helps with annual licensing preparation. They partner with the leadership team on staff communications and are responsible for coaching, mentoring, training, and overseeing the performance and career progression of direct reports. This position also serves as a backup for the positions supervised.

Requirements

  • Associate’s Degree in human services, social work, psychology, or related field (OR) a High School Diploma or equivalent and two (2) years of experience
  • Demonstrates interest in the development of others

  • Valid driver’s license

Responsibilities

  • Direct and oversee daily program operations within assigned residential group homes through the supervision of team members, including performance evaluations and management, scheduling, and ODP staff compliance
  • Responsible for visiting, at a minimum, one group home per day and will visit all assigned residential group homes within their responsibility at least once a week
  • Under the supervision of the SSM, participate in interviews, selection, and the onboarding process of new hires
  • Ensure services are provided in accordance with agency policies and procedures
  • Direct and oversee medication management, electronic health record documentation and monitoring, medical training for staff regarding individual’s diagnoses and medical conditions, and staff support for health/medical related questions
  • Coordination of medical appointments for individuals including transportation and follow-up
  • Facilitate effective communication within the team, including external individual supports
  • Participate in employee relations activities with the team and engage in disciplinary actions, as needed, with assistance from Human Resources
  • Assist with annual licensing preparation and plans of corrections as needed
  • Partner with leadership team to manage the communications for staff and leadership including annual enrollment guides, new hire guides, etc.
  • Coach and mentor direct reports; provide training, oversee performance, and aid with career progression
  • Perform other related duties and assignments as directed by the Support and Services Manager
  • Serve as the backup for the positions supervised
  • Perform basic math functions
  • Write, read, and comprehend what is read
  • Utilize expressive and receptive language skills
