About The Position

We're looking for a dedicated Home Care Specialist to lead the intake, assessment, and ongoing management of care for our clients. In this role, you are the bridge between families, caregivers, and our office team. From the first intake call to developing individualized Plans of Care, you ensure every client receives safe, personalized, and high-quality service. What You'll Do: Handle intake calls and guide families through care options Conduct in-home assessments and complete admission paperwork Develop and manage individualized Plans of Care Perform supervisory visits and support caregiver performance Address concerns, complaints, and incidents professionally Participate in on-call rotation and step in when coverage is needed Maintain compliance with all regulations and company standards This role requires organization, emotional intelligence, and the ability to stay calm under pressure. You'll work independently while collaborating closely with scheduling and leadership to ensure continuity of care.
