About The Position

Eddy Visiting Nurse & Rehab Association offers a rewarding full-time opportunity for a Registered Nurse to join their home care team in Saratoga County. Requirements

• 1 year prior experience in acute care, rehab, or homecare
• Organized
• Able to teach
• Willing to travel from patient to patient
• Strong communication with other interdisciplinary team members and family members
• Strong critical thinking skills
• Valid NYS RN Licensure
• Valid DMV Licensure

Responsibilities

• Work directly with an interdisciplinary team in the homecare setting
• Travel from patient to patient
• Maintain strong communication with other interdisciplinary team members and family members
• Care for one patient at a time
• Address the whole patient
• Help patients become independent with their treatments and self-management of their chronic illness(es)
