About The Position

Are you passionate about making a difference in patients' lives as they transition from hospital to home care or hospice services? Trinity Health At Home is seeking a dedicated Home Care/Hospice Coordinator to join our team and help ensure seamless, compassionate care for our clients. Home Care. Around 8 hours a week, Contingent. Sundays and possibly Saturdays. Covering Fresno and Iowa referrals. Home Care Coordinator-Contingent Job Summary The Home Care & Hospice Coordinator is responsible for ensuring patients in the acute care setting are efficiently referred to appropriate homecare or hospice services. This role provides education to physicians, nurses, and other hospital staff about available services and may support business development initiatives, new program development, and expansion efforts. Supervision None

Requirements

  • Bachelor’s degree or equivalent experience required.
  • Completion of an approved clinical education program and current clinical license preferred.
  • At least two years of experience in an acute care setting, customer service environment, sales/marketing role, or related field.
  • Valid driver’s license and reliable transportation.
  • Strong written and verbal communication skills.
  • Ability to adhere to the organization’s mission, Code of Ethics, Compliance Program, and professional standards.

  • Completion of an approved clinical education program and current clinical license preferred.

Responsibilities

  • Maintain working knowledge of all relevant Federal, State, and local regulations, insurance coverage requirements, and agency policies.
  • Adhere to ethical, professional, and compliance standards in all activities.
  • Maintain ongoing communication with case managers, discharge planners, and hospital units to ensure timely follow‑up on referrals.
  • Answer questions from physicians, hospital staff, patients, and families regarding homecare, hospice, and community‑based services.
  • Coordinate care for patients leaving the hospital who select the agency’s services.
  • Gather and review medical record information and document initial assessments according to policy.
  • Arrange services based on established criteria, clinical needs, and patient preferences.
  • Obtain physician orders and ensure compliance with agency and payer requirements.
  • Use interviews, observation, and clinical judgment to assess needs, recommend services, and support care planning.
  • Communicate with healthcare team members, patients, and families to provide education and ensure clarity about available services.
  • Refer patients to other services when appropriate and coordinate with providers to ensure effective care.
  • Send required patient information to agency intake electronically or manually.
  • Notify hospitals of patient readmission and provide status updates.
  • Accurately document assessments, interventions, and continuing care management in compliance with agency policies.
  • Submit required patient data promptly via electronic transmission, fax, or mail.
  • Participate in quality improvement activities and use Continuous Quality Improvement principles in daily work.
  • Assist in developing and implementing new programs or service initiatives.
  • Initiate insurance verification and confirm homecare benefits or provide information to the intake coordinator.
  • Explain insurance coverage and financial responsibility to patients and families.
  • Meet monthly or quarterly sales or referral targets.
  • Build and maintain positive working relationships with hospital discharge planners, physicians, and clinical staff.
  • Maintain effective communication across teams, departments, and provider groups.
  • Participate in case conferences when needed.
  • Visit physicians and providers to educate them on agency services and establish referral relationships.
  • Seek consultation and mentoring from supervisors and clinical experts.
  • Maintain currency in clinical and industry best practices.
  • Share knowledge with colleagues and participate in orientation or education of others when requested.
