About The Position

Parx is looking for a Home Care Coordinator to join our team! The Home Care coordinator will be instrumental in improving the quality of life for those requiring non-skilled care while allowing them to preserve their independence and dignity in their own homes. The Home Care coordinator evaluates, plans, and coordinates to result in a favorable health outcome for every client. This role is responsible for ensuring that clients receive the appropriate care tailored to their individual needs, coordinating services, and acting as the liaison between clients, families, healthcare providers, and agency staff. The Care Coordinator will also be responsible for maintaining accurate records, assessing clients’ needs, and ensuring compliance with Medicaid regulations. Coordinating schedules between non-skilled patients and their assigned caregivers while ensuring the EMR reflects those schedules with appropriate authorization.

  • Experience with healthcare coordination, scheduling, or insurance (preferred)

Responsibilities

  • Manage patient referrals, verify information, and handle insurance/authorization processes
  • Communicate with patients and families, providing updates and guidance throughout care
  • Enter and maintain accurate patient and insurance data in the system
  • Create and update care plans in compliance with Medicaid guidelines
  • Coordinate caregiver schedules based on authorized hours and client needs
  • Monitor Medicaid eligibility and ensure services meet coverage requirements
  • Collaborate with internal teams, case managers, and healthcare providers
  • Build strong relationships with clients, families, and referral sources
  • Respond to urgent needs and support overall patient care coordination
  • Assist with staff compliance, training, and general administrative tasks
