The HIV Care Coordination Program (CCR) addresses HIV healthcare disparities by facilitating access to care and other services through a client-centered, holistic, and comprehensive approach to meeting the needs of persons through team-based care management. The program goals include prompt linkage and ongoing retention in HIV care, optimal treatment adherence, viral suppression, and reduced HIV-related mortality. This is achieved through activities such as patient navigation, directly observed therapy, health education, and coordination with the medical team; the frequency of support is driven by the intensity of an individual’s need. The program is community-based and requires in-person activity. The Navigator for Special Populations specializes in engaging priority populations, including men who have sex with men (MSM), transgender individuals, and those experiencing mental health or substance use challenges. Candidates with lived experience, such as personal or community-based experience with HIV, behavioral health, or navigating systems as part of BIPOC or LGBTQ+ communities, are encouraged to apply. This perspective helps build trust, promote equity, and deliver culturally responsive care. The Navigator supports clients in achieving program goals such as linkage to care, retention, treatment adherence, and viral suppression through activities like patient navigation, directly observed therapy (DOT), health education, and coordination with medical providers. The role is community-based and requires in-person engagement.
