About The Position

As the are Coordinator for Healthcare Home (HCH), you would be responsible for assisting with the coordination of HCH activities. This will include maintaining, tracking, and entering of all screening data, and the completion of reporting. You would also provide general clerical support to the HCH Team as appropriate.

Requirements

  • High School Diploma, GED, or HSE required
  • Must be proficient in Microsoft Office
  • This position requires a valid driver's license, proof of continuous auto insurance, and must have an acceptable driving record as outlined by agency policy.
  • Utilizing your solid Customer Service skills such as compassion, empathy, positive demeanor, effective communication, active listening, adaptability, attentiveness, patience, self-control, conflict resolution and de-escalation skills are necessary and will be always provided to our clients and staff.
  • Time management, organization skills, accountability, accurate documentation, adhering to healthy professional boundaries & maintaining true to CCC ethics and policies will provide structure and success to your day.
  • Along with providing exceptional client care, protecting our client's information is of the utmost importance and will be required, as we adhere to all HIPAA laws and regulations.

Responsibilities

  • Provides general clerical support to the HCH Team as appropriate.
  • Provides assistance with coordination letters and metabolic syndrome results to the team, CSC and the Primary Care Physicians.
  • Completes data entry of all metabolic syndrome screening results into CareManager.
  • Serves as a CyberAccess Practice Administrator responsible for maintaining user accounts.
  • Provides technical assistance to team and CSCs on use of Cyber Access and CareManager.
  • Assists with tracking all required client screenings and reporting measures are complete and submitted as appropriate.
  • Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards
