JOB SUMMARY: Responsible for the development and implementation of home-based services for program participants. Oversees the work and schedules of the PACE floater aides and assists the Center Manager in overseeing the work of the day center aides. Serves on the interdisciplinary team and performs administrative tasks related to direct operations, especially care planning. JOB RESPONSIBILITIES: Assesses the home care needs of frail elderly participants through initial, semiannual and unscheduled assessments and develops specific plans of care to address identified needs. Coordinates the implementation of home care services, including personal care, homemaker and respite through contracts with agency providers. Assists in the coordination and oversight of home-based services including nursing and therapy. Supervises and oversees the work schedule of the PACE floater aides to ensure adequate home care coverage for program participants. Performs supervision of the work performed by the PACE floater aides. Assists the Center Manager in overseeing the work of the day center aides, particularly in the event of Center Manager absence. Serves on the Interdisciplinary Team (IDT) and assists in the development of care plans to address participants’ individual needs. Documents assessments and relevant home care information in the electronic health record. Serves as the liaison for contracted home care agencies, reporting any concerns reported to the IDT in a timely manner. Performs home assessments related to reports of pests and/or issues that may pose a safety risk. Serves as the liaison for individuals responsible for pest management. Performs home assessments, as indicated, related to reports of falls. Assists in the provision and tracking of durable medical equipment (DME) issued to participants. Assists in determining home modification needs and acts as the liaison for home modification agencies. Reviews and ensures accuracy of home care billing reports from outside agencies and requests corrections as needed. Ensures timely receipt of home care records to ensure completeness of the EHR in accordance with federal and state guidelines. Provides administration on call support in rotation with other members of the administrative team. Serves on the PACE Administrative Team and assists in the creation and modification of policies and procedures. Participates in the Quality Improvement program. Approves employee payroll, mileage entry and leave requests as appropriate. Participates in the coordination of 24-hour care delivery system. Completes all documentation in the medical record as required. Acts only within the scope of his or her authority to practice. Performs other job-related duties as directed by PACE Director.
