MJHS is a large not-for-profit health system in the Greater New York area. Our range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care at Menorah and Isabella Centers, and the research based MJHS Institute for Innovation and Palliative Care. Requirements

  • Bachelor’s Degree, required; BS in health care related field highly preferred
  • Home Care or Health Care experience preferred.
  • Specialized knowledge in Microsoft Office, (Word, Excel, Outlook, etc.)

Responsibilities

  • Evaluates patients referred for home health services.
  • Manages and facilitates the transition of patients onto the home care program in accordance with applicable law, regulations and agency policies and procedures under the direct supervision of a Home Care Planner (HCP) Manager and/or Director of Business Development.
