About The Position

The Health Program Coordinator I will coordinate the New York State Department of Health’s (NYSDOH) Building Our Largest Dementia Infrastructure (BOLD) Program. This position will work within the NYSDOH Alzheimer’s Disease Program, with responsibilities that include the management of federally funded agreements; engaging subcontractors and facilitating the implementation of strategies in the healthcare setting and community-based setting; contributing to the development of the subcontractor's scope of work and budget; providing guidance and technical assistance in removing identified barriers to implementing program strategies; hosting and facilitating stakeholder meetings; hosting and facilitating monthly internal/external project team calls; contributing to monitoring activities; and developing program specific presentations, website content, and related workplan activities. Other appropriate related duties as assigned. The Alzheimer’s Disease Program currently funds a comprehensive network of programs and services that support caregivers and those diagnosed with Alzheimer’s Disease or related dementia (ADRD) in their communities. The Alzheimer’s Disease Caregiver Support Initiative, established in 2015, is based upon research demonstrating that access to appropriate support services will lead to improved caregiver outcomes, reduced caregiver burden and a delay in institutionalization. This includes removing barriers to accessing social support services aimed at addressing social determinants of health.

Requirements

  • A Bachelor's degree in a related field and three years of experience in program coordination of a public health, health regulatory, or human services related program; OR an associate degree in a related field and five years of such experience; OR seven years of such experience.
  • A Master's degree in a related field may substitute for one year of such experience.
  • At least one year of experience must have included supervision of staff and/or program management.
  • Grant funded position. Compliance with funding requirements such as time and effort reporting, grant deliverables, and contract deliverables, is required.
  • Valid and unrestricted authorization to work in the U.S. is required.
  • Visa sponsorship is not available for this position.
  • Travel up to 10 % of the time will be required.
  • The selected candidate must reside within a reasonable commuting distance of the official work location specified in the job posting and must also be located in or willing to relocate to one of the following states: New York, New Jersey, Connecticut, Vermont, or Massachusetts, prior to hire.
  • Telecommuting will be available.

Nice To Haves

  • Graduate degree in public health, public/health care administration, social work, nursing, or related field.
  • Professional experience working with individuals living with Alzheimer’s disease and related dementia.
  • Experience with convening and facilitating a learning collaborative or advisory/stakeholder group using quality improvement methodology in collaboration with clinical and community partners.
  • Experience in the management of state or federally funded contracts; experience with grant/contract procurement; and, providing training or technical assistance to health systems and/or community-based organizations in strategy implementation.
