Summary Statement This position provides support to the Family Resource Coordinator for individuals with intellectual/developmental disabilities enrolled in the Medicaid Home and Community Based 1115 Waiver and eligible for DDDS services. Responsibilities include: coordinating with families and their respective Managed Care Organizations (MCO) provider to ensure individual's needs are met, ensuring quality services are being provided, maintaining case records, and establishing effective working relationships with key stakeholders. Routine travel to provider sites is required, along with occasional emergency transportation for individuals. An employee in this class may participate as a member of a multi-disciplinary team to implement behavioral health services or treatment plans in accordance with Delaware Code, Title 24.
