Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. Position Summary: We have an exciting opportunity to join our team as a Health Educator. In this role, the successful candidate The health educator will facilitate evidence-based sexual health workshops for adolescents in a high school setting, implement risk reduction counseling and provide sexual health education, and facilitate peer education group at Project Reach Youth site.

Requirements

  • HS Diploma
  • Prior work experience with community health outreach.
  • Health related background.
  • Qualified candidates must be able to effectively communicate with all levels of the organization.

Nice To Haves

  • At least three years providing services to young adults
  • BA
  • Good interpersonal skills
  • Workshop facilitation experience
  • Interest in public health/sexual health education
  • HIV testing experience
  • Experience with electronic medical record system (EPIC)
  • Second language: Spanish, Haitian Creole, Mandarine, Arabic

Responsibilities

  • Work with Program Coordinator and school staff to schedule workshops in high schools, middle schools, and other schools in CBOs in Brooklyn.
  • Conduct evidence-based sexual health workshops using Making Proud Choices curriculum..
  • Administer sexual health risk assessment surveys to all workshop participants
  • Meet individually with workshop participants identified as high risk to provide risk reduction counseling connect them to the FHC School Based Health Center or other social services.
  • Co-facilitate sexual health education peer education group, organize weekly group planning meeting, submit sessions plans, ensure attendance is kept up to date.
  • Supervise senior assistant assigned to your group
  • Meet quarterly with each peer educator in your group for check-ins and provide referrals as needed.
  • Provide rapid HIV counseling at FHC School Based Health Centers and complete all related documentation.
  • Participate in project SAFE community events to connect youth STI and HIV testing.
  • Participate weekly staff meetings at Project Reach Youth 1
  • Submit workshop attendance information in ETO 1
  • Participate in SBHC morning huddle and other meetings as needed.
