Health Care Navigators engage in proactive outreach to build successful working relationships between members and healthcare professionals; helping to support, engage, and refer members to available programs, resources, and services to navigate the complex health care system. They work effectively with our healthcare professionals “Care Teams” to empower members and their families towards positive change and participation in their health.

Requirements

  • Must reside in the Kansas City Metro
  • Requires two years (60 credit hours) of college. Additional years of qualifying work experience may be considered in lieu of formal education
  • Ability to work as part of a multidisciplinary team
  • Requires effective written and verbal communication skills
  • 3 years’ experience in a clinical setting, human services, care management or healthcare call center
  • Must have the ability to provide a non-cellular High Speed Internet Service such as Fiber, DSL, or cable Modems for a home office. A minimum standard speed for optimal performance of 30x5 (30mpbs download x 5mpbs upload) is required. Latency (ping) response time lower than 80 ms
  • Hotspots, satellite and wireless internet service is NOT allowed for this role.
  • A dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information

Responsibilities

  • Engage Member: Conduct outreach to member population to introduce and engage members to participate with Population Health Management team programs.
  • Receive inbound Care Team phone calls or emails and address or route appropriately.
  • Assesses the readiness and level of participation of members to engage in care management services.
  • Refer to RN/program Collaborate with Care Team utilizing appropriate tools to prioritize workload and transfer member to RN if possible, or schedule appointment.
  • Identify member needs and refer to appropriate internal and external programs.
  • Redirect providers and members to network facilities when available.
  • Perform Supportive Tasks Confirm member eligibility, obtain general member information, verify demographics, and provide non-clinical information prior to the transfer of the member to the RN.
  • Perform supportive tasks for members or care team such as sending letters, member education materials and provider lists.
  • Assist members with contacting providers for appointments, community agencies, other care management professionals and appropriate GEHA resources.
