Minimum Qualifications: Level I: Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC) Two years of professional nursing experience Level II: Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC) Three years of professional nursing experience Job Responsibilities Assist with identification of DDS service recipient's health risk by: Completing a Physical Status Review (PSR) health acuity tool; Analyzing medical documentation; and Participating in service recipient's interdisciplinary team meetings to assist with development of the Individual Plan. Conducts home visits and reviews the home medical record for service recipients determined by PSR acuity score to have elevated health needs/risks. Updates medical records in consumer's electronic file. Develop a Nursing Service Support Plan as indicated. Conducts telephone calls with hospital medical staff on behalf of service recipients during hospital admissions to obtain updates on the individual's condition and to assist with planning for safe discharge. Completes initial 24 Hour Mortality Report when notified of service recipient’s death. May complete a Pre-admission Screening and Resident Review (PASRR) assessment for determination of admission eligibility to a Medicaid certified nursing facility for persons who may have an intellectual disability. Requirements

  • Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)
  • Two years of professional nursing experience
