Minimum Qualifications: Level I: Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC) Two years of professional nursing experience Level II: Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC) Three years of professional nursing experience
Requirements
- Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)
- Two years of professional nursing experience (Level I)
- Three years of professional nursing experience (Level II)
- Must possess a valid driver's license and must maintain required car insurance.
Responsibilities
- Assist with identification of DDS service recipient's health risk by: Completing a Physical Status Review (PSR) health acuity tool;
- Analyzing medical documentation; and Participating in service recipient's interdisciplinary team meetings to assist with development of the Individual Plan.
- Conducts home visits and reviews the home medical record for service recipients determined by PSR acuity score to have elevated health needs/risks.
- Updates medical records in consumer's electronic file.
- Develop a Nursing Service Support Plan as indicated.
- Conducts telephone calls with hospital medical staff on behalf of service recipients during hospital admissions to obtain updates on the individual's condition and to assist with planning for safe discharge.
- Completes initial 24 Hour Mortality Report when notified of service recipient’s death.
- May complete a Pre-admission Screening and Resident Review (PASRR) assessment for determination of admission eligibility to a Medicaid certified nursing facility for persons who may have an intellectual disability.
- Assist with seeking out existing generic community health related services to meet the service recipient’s health needs and actively develop (locate, contact and identify) services when none are available.
