About The Position

Ideal candidates must be willing to work non-traditional hours such as evenings and mornings to meet the needs of the individuals receiving services. Essential Functions An employee in this classification is required to perform some or all of the following duties, however these do not include all of the tasks which the employee may be expected to perform: provide direct case management services to persons served (coordination, linking and monitoring of services); conduct and document assessment of needs of persons served; maintain accurate and timely records of persons served; develop goals and objectives in cooperation with persons served/family/support system and/or guardian, and other team members for appropriate plan; consistently implement and follow Person Centered Planning process; provide observation and monitoring of service/goal implementation and recommend improvement opportunities; develop community connections and coordinate/link with schools, physicians within the community, courts, or other programs/agencies within the community; comply with Transportation Policy (09-002-0010), Alcohol & Drug Testing Policy (06-001-0010) and Background Check Policy (06-001-0015), as well as supervisors/designee directives; maintain confidentiality. This position receives supervision from program supervisor.

Requirements

  • Bachelor’s Degree in Social Work, Education, Special Education, Health Education, Health Administration, Psychology, Human Services, Criminal Justice, or related field relative to position
  • Valid driver’s license
  • At least 1 to 2 years’ experience working with children in job related field or evidence of such placements during schooling
  • Demonstrated ability to use Microsoft Office applications and Windows 10 programs
  • Must have access to transportation
  • Must qualify as QMHP, QIDP, or CMHP, according to program assignment
  • If using agency vehicles, passing random drug and alcohol screening
  • Ability to use some flexibility in problem solving, using discretion in selecting an optimal solution from among established alternatives with a clear outcome.

  • Using or exerting influence in a work process and be a “work leader” or advise others
  • Ability to provide, exchange, or explain information which conveys facts, opinion or evaluation of the facts or analyses

Nice To Haves

  • Licensed Bachelor Social Worker
  • Three (3) to five (5) years of experience in social work-related field is preferred.
  • Demonstration of proficiency and experience working on an electronic health record system
  • Knowledge of CMH system and operations
  • Lived experience with behavioral health issues
