About The Position

JOB SUMMARY: The primary purpose of the Admissions Nurse RN is to evaluate and admit patients to the organization’s services in accordance with established policies, regulatory requirements, and professional standards of nursing practice. This Nurse conducts comprehensive assessments, determines patient eligibility for services, explains care options, obtains required consents, and initiates individualized care plans that promote safe and effective transitions into care. It also serves as the first clinical contact for new patients and families, ensuring that admissions are completed accurately, efficiently, and with compassion. This position requires an in-depth understanding of clinical criteria, regulatory compliance, and the ability to educate patients and caregivers about available services and expectations of care.

Requirements

  • Associates or Bachelors degree in nursing required.
  • Must have three to five (3-5) years’ experience in nursing, preferably in long term care.

Nice To Haves

  • Admissions experience also preferred.

Responsibilities

  • Reviews medications on referrals and performs an analysis regarding availability of medications and clinical capabilities.
  • Transcribes physician’s orders and queries the physician as needed to ensure accuracy of resident medical information prior to admitting the resident.
  • Reviews and evaluates resident diagnoses and overall condition related to criteria for skilled coverage as well as related to medical eligibility criteria for long term care placement prior to admitting the resident.
  • Assists with processing referrals timely on a daily basis as assigned by the Admissions Director.
  • Identifies and documents primary and other diagnoses codes for all admissions and readmissions in the medical record based on a review of the available medical information.
  • Informs the Admissions Director of obstacles or barriers to admitting residents and recommends solutions to such obstacles and barriers.
  • Communicates with interdisciplinary team members timely regarding clinical needs such as equipment, medications, etc. for pending admissions.
  • Speaks effectively with physicians when necessary to clarify medical information
  • Participates in offsite referral patient assessments as necessary.
  • Facilitates completion of all required clinical assessments for New Admits.
  • Performs skin audits, order audits, referral reviews.
  • Serves as the Admissions Coordinator backup and interacts with referral sources.
