Make an impact every day by coordinating services, guiding individuals through treatment, and supporting their progress toward stability, independence, and community reintegration within a structured therapeutic residential setting. The Forensic Case Manager provides comprehensive case management services to ORLTSR residents, ensuring they receive coordinated, person-centered support throughout their stay. This role is responsible for developing individualized care plans, linking residents to community and clinical services, monitoring progress, and facilitating communication between residents, treatment teams, and external agencies. The Forensic Case Manager plays a critical role in helping residents achieve treatment goals, comply with court or conditional release requirements, and successfully transition to less structured levels of care.

Requirements

  • Bachelor's degree in mental health, social work, or related field
  • One year mental health and forensic experience
  • Current Driver's License; satisfactory driving record; eligible to operate program vehicles.
  • Works well in a collaborative environment; having the ability to use creativity with problem solving and adapt to changing priorities in a fast-paced environment
  • Strong clinical skills and knowledge of the behavioral health system

  • Experience working with individuals with serious mental illness, behavioral health needs, or justice involvement preferred but not required—COMHAR provides training.

Responsibilities

  • Provide care management services, therapeutic interventions, and trauma-informed, culturally competent support within a recovery-oriented framework.
  • Assist residents in identifying and achieving individualized recovery goals through a strengths-based approach.
  • Coordinate and follow up on all services related to Northampton County competency hearings and mental health court hearings, including communication with relevant parties.
  • Acquire, coordinate, and monitor all services necessary for residents to attain their treatment goals.
  • Supervise resident fund management and ensure accountability in financial matters.
  • Advocate on behalf of residents with internal and external programs, agencies, and service providers.
  • Collaborate with the Director and Assistant Director on admission screenings, referral negotiations, transfers, and discharges.
  • Maintain accurate and timely documentation, ensuring compliance with all internal policies, external regulations, and court-related requirements.
  • Participate in treatment team meetings and collaborate with psychiatrists, therapists, nurses, peer specialists, and other staff to ensure integrated, person-centered care.
  • Provide crisis intervention and support as needed, following established safety protocols.
  • Promote a recovery-oriented, respectful, and supportive environment that fosters independence, skill development, and successful community reintegration.
