About The Position

Requirements

  • Requires a Bachelor’s Degree in either forensic psychology, social work, counseling or a related field; a license in social work (LSW) or counseling (LAC), is preferred.
  • A genuine desire for forensic clinical services and a comfort in moving about in the County jail system and the community.
  • Valid driver’s license.
  • Able to work a flexible schedule, including evening, weekend and holiday hours.

Responsibilities

  • Provide effective discharge planning, linkage and referral to community resources for mentally ill offenders re-entering the community following release from the county correctional system and jail diversion from local municipalities. Service includes the provision of holiday, weekend and on-call coverage as assigned.
  • Provide mental health training to municipal workers, as needed.
  • Assist the Program Director in project supervision, report preparation and meeting participation.
  • Record keeping as required by funding sources through the electronic clinical record software, and/or accreditation agencies, including the development and implementation of a discharge plan and completion of other documentation as required.
  • Facilitation of access to appropriate services, including transportation to services and activities as necessary and application for and receipt of all applicable public entitlement.
  • Coordination and integration of services from multiple providers when the client is discharged from the county jail system. This responsibility may include coordination of meetings of the client’s service providers in the community.
  • Monitoring of service delivery to meet a client’s changing needs and advocacy as necessary. Direct Care Staff must meet all expected productivity levels, in regard to direct service provision to consumers.
  • Identification of client resource gaps and problems of service delivery and provision of direct service support to the client’s natural support system, including family,
  • friends, employers and self-help groups.

  • Perform other duties as assigned by the Program Director.
