The Case Manager ensures a safe and supportive environment for patients throughout treatment. This role assists with orientation, admissions, discharge planning, and documentation, while serving as a liaison between patients, the clinical team, and admissions staff.

Requirements

  • Knowledge of addiction and recovery preferred
  • Knowledge of CARF, DCF and Joint Commission Standards, preferred
  • Strong interpersonal and communication skills for working with patients, families, and interdisciplinary teams.
  • Knowledge of mental health conditions, treatment modalities and community resources.
  • Bachelor’s degree in social work, psychology, counseling or related human services field preferred.
  • Prior experience in behavioral health, mental health, or substance use treatment setting preferred.
  • Current CPR Certification
  • Crisis Prevention Intervention (CPI) Training
  • Successful clearance of a Level II background check

Responsibilities

  • Conduct comprehensive assessments to identify clients' needs and develop individualized care plans.
  • Coordinate and facilitate access to various services, including therapy, medical care, and community resources.
  • Monitor clients' progress and adjust care plans as needed, ensuring effective treatment and support.
  • Provide ongoing support and counseling to clients, helping them navigate challenges and maintain motivation.
  • Collaborate with healthcare professionals, family members, and community organizations to ensure a cohesive support system.
