About The Position

We are seeking a dedicated and compassionate Forensic Case Manager to join our team. This role is responsible for providing case management and support to justice-involved individuals with mental health and/or co-occurring disorders. The ideal candidate is reliable, organized, and client-focused, with strong knowledge of the criminal justice system, community resources, and the ability to work effectively in a fast-paced, dynamic environment. The Forensic Case Manager provides individualized case management and support to individuals with serious mental health diagnosis involved in the criminal justice system who are receiving mental health and/or co-occurring disorder services. This role ensures that clients receive comprehensive care, including treatment coordination, advocacy, and linkage to community resources, while monitoring compliance with court or legal requirements. The ideal candidate is organized, empathetic, and experienced in working with justice-involved populations, with strong communication and problem-solving skills.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Criminal Justice, or a related field required; Master’s degree preferred
  • One year mental health and forensic experience
  • Current Driver's License; satisfactory driving record; eligible to operate program vehicles.
  • Experience working with justice-involved populations, mental health, and/or co-occurring disorders
  • Knowledge of the criminal justice system, court processes, and community resources

Nice To Haves

  • Bilingual proficiency in English and Spanish, with strong reading and writing skills is preferred but not required.

Responsibilities

  • Conduct assessments to identify client needs, strengths, and risks in collaboration with the treatment team
  • Develop and implement individualized case plans that address mental health, substance use, housing, employment, and other recovery-related goals
  • Monitor client progress and compliance with court-ordered conditions and treatment plans
  • Advocate on behalf of clients with courts, probation/parole officers, treatment providers, and community agencies
  • Facilitate connections to community-based services, including housing, vocational programs, education, and healthcare
  • Provide crisis intervention and support as needed
  • Maintain accurate and timely documentation of client interactions, progress, and outcomes
  • Participate in multidisciplinary team meetings to coordinate care and support for clients
  • Educate clients about their rights, responsibilities, and available resources
  • Support clients in building skills for independent living, self-advocacy, and positive social connections
  • Ensure compliance with all organizational policies, confidentiality requirements, and applicable regulations
  • Participate in quality improvement initiatives, staff training, and professional development.
