About The Position

Provide support to clients/residents with mental health needs who have been incarcerated at the county, state, or federal level to integrate into transitional housing to offer a choice between custody and independence.

Requirements

  • High School Diploma, GED, or an equivalent combination of skills, experience, and education may be considered at the discretion of the Agency.
  • Ability to receive, understand, and communicate information with effectiveness and follow instructions.
  • Must have a valid Pennsylvania Driver’s License and a safe driving record.
  • Must have the ability to utilize electronic medical records.
  • Reliable, organized, and committed to the health and safety of the clients.
  • Displays a positive and compassionate attitude and manages multiple tasks at one time.
  • Must successfully obtain Criminal History Clearance from the PA State Police and Child Abuse Clearance from the PA Department of Human Services, and FBI Clearance.
  • Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program.

Responsibilities

  • Provides hands-on instruction to residents to implement Individual crisis plans and assist with crisis resolution.
  • Provide support by aiding clients to live as independently as possible and reach obtainable goals to maintain their independence.
  • Aids and support with the resident's Activities of Daily Living and Instrumental Activities of Daily Living as indicated in the resident’s assessment and support plan.
  • On a day-to-day basis, support with meal preparation, household chores, medication management, budgeting and finances, accessing community facilities/activities, shopping, socialization, doctor’s appointments, and providing emotional support.
  • Assist the client with skill development and job placement.
  • Monitor residents' progress and complete documentation, identifying problem areas that may need to be addressed.
  • Participates in regular staff training to include specialized training, to develop basic knowledge and skills to assist residents in crisis resolution, and support job performance.
  • Responds appropriately to emergency situations.
  • Maintains proper and detailed documentation of all assistance and support provided to each resident.
