Join our Forensic Assertive Community Treatment Teams (FACT) in providing comprehensive, integrated care to adults living with mental illness, substance use disorder, and associated medical conditions. Team services are provided where and when clients need them—in their homes, at work, and in other community settings. Team members stay in close contact with clients to help them recover and to respond quickly with more services and supports if circumstances change. The Mental Health Practitioner Vocational Specialist will use an Individual Placement and Support (IPS) model to help people find, get and stay involved in educational or employment opportunities. Training in the model is available. Using this lens, the Vocational Specialist integrates trauma informed, clinical and psychiatric rehabilitation interventions. They provide cross trainings and guidance to the team to facilitate the team's collective effort to support achievement of vocational goals. Additional job responsibilities include helping people access and maintain benefits and community resources, providing care coordination with external providers, connecting with natural support and providing crisis intervention services.
