Job Summary:The Support Coordinator helps individuals with disabilities and their families to identify and link with needed services and supports to improve their quality of life within the home and the community. As a Support Coordinator, you must be willing and able to travel and drive throughout the State of NJ as well as have a flexible schedule to accommodate consumers’ schedule including evening and weekend hours.

Requirements

  • Bachelor’s Degree in a relevant discipline and 1-year equivalent experience working with individuals with intellectual or developmental disabilities.
  • Computer Proficient (MS Office Outlook, Word, Excel, PowerPoint).
  • Able to solve problems independently, effectively, and creatively.
  • Excellent Organization and Communication skills.
  • Excellent time management skills to be able to juggle multiple plans for multiple families simultaneously.
  • Balance of work in the field and remote office- must have a flexible schedule to meet the needs of our individuals.
  • Valid Driver’s license and reliable car to be able to travel throughout the state of NJ.

Responsibilities

  • Provides caseload coverage throughout New Jersey, when we require a Support Coordinator’s caseload to be maintained for a period of time.
  • Travels to in person meetings throughout the state; attends quarterly visits, planning team meetings, and plan signatures.
  • Works alongside our internal program trainer and community liaison on overall program growth regarding training, presentations, and webinars.
  • Supports our enrollment team; inputs needed information into electronic I-Record database.
  • Maintains and develops individual plans of care, with input from the individual and family, which is based on the personal preferences and choices of the individuals being served.
  • Utilizes available specialized and generic community resources in combination with other formal and informal supports and services.
  • Maintains continuing contact with individuals and families as individual plans of care are put in place.
  • All support coordination deliverables as outlined in the Policy & Procedure Manuals (SP & CCP).
  • Assist individuals and their families in coordinating, selecting, and overseeing their own supports.
  • Continues learning about relevant, local accessible services and providers.
  • Informs providers and the Division about identified service gaps.
  • Completes required training and attends meetings as assigned.
  • May perform other duties as assigned by the CEO, SVP of Programs and Services, Director, SC Supervisor, and Administrative Assistant as need arises.
