About The Position

The Field Supervisor is responsible for overseeing the delivery of high-quality in-home care services by home health aides (HHAs), CNAs, and caregivers. This role ensures compliance with agency policies, state regulations, and care plans through regular field visits, caregiver supervision, client assessments, and performance management. The Field Supervisor serves as a key liaison between clients, caregivers, and office staff to ensure continuity, safety, and quality of care.

Requirements

  • Licensed Practical Nurse (LPN), Registered Nurse (RN), or equivalent clinical/field supervisory experience (state-specific requirements apply).
  • Minimum 2 years of experience in home healthcare, long-term care, or related healthcare setting.
  • Knowledge of state home care regulations , caregiver supervision standards, and care plans.
  • Valid driver’s license and reliable transportation.
  • Ability to travel regularly to client homes.

Nice To Haves

  • Prior experience supervising HHAs, CNAs, or direct care staff.
  • Experience with electronic health records (EHR) or home care management systems.
  • Strong conflict resolution, documentation, and organizational skills.

Responsibilities

  • Caregiver Supervision & Support
  • Conduct routine and as-needed field visits to clients’ homes to supervise caregivers.
  • Observe care delivery to ensure services are provided according to the care plan , agency standards, and regulatory requirements.
  • Provide coaching, training reinforcement, and corrective guidance to caregivers.
  • Address caregiver performance concerns and participate in disciplinary actions when necessary.
  • Verify caregiver compliance with documentation, timekeeping, and visit verification systems.
  • Client Care & Quality Assurance
  • Perform initial, routine, and reassessment visits to evaluate client needs and satisfaction.
  • Identify changes in client condition and communicate promptly with clinical leadership.
  • Ensure client safety, dignity, and quality of care are consistently maintained.
  • Respond to client concerns, complaints, or incidents in a timely and professional manner.
  • Compliance & Documentation
  • Ensure all supervision visits, assessments, and follow-ups are accurately documented in accordance with state regulations and agency policy.
  • Assist with state audits, surveys, and internal quality reviews as needed.
  • Support compliance with training requirements, competencies, and licensure/certification standards.
  • Coordination & Communication
  • Act as a liaison between caregivers, clients, families, and office staff.
  • Collaborate with scheduling, HR, and clinical teams to ensure appropriate caregiver placement.
  • Participate in care conferences, staff meetings, and quality improvement initiatives.
