About The Position

We are looking for an experienced and motivated RN home healthcare director to join our new home care agency. About us: Foxwoods Home Care, LLC is a newly licensed home care services agency providing care to clients within their home. Our agency will operate at Canterbury Woods (both the Williamsville and Buffalo locations) and at Fox Run (Orchard Park). We have one goal and one commitment: We provide older adults with the best possible living experience. Home care is specific to each client’s needs and provided in their home at a scheduled time. Care is provided to support and maintain the physical and emotional well-being of each client. Person-centered care takes each client’s individual preferences, choices, dignity, interest, and capabilities into consideration. RN Home Healthcare Director position details: We are hiring for a Full Time (5 days/week) position.

Requirements

  • Must have credentials as an RN with a current New York State license.
  • Must be CPR and first aid certified.
  • Must have verbal and written English skills.

Responsibilities

  • Primarily responsible for the operation and management of the agency.
  • Ensure that the agency operations and standards comply with all local, state, and federal rules, regulations, and statues.
  • Maintain quality of care standards within prevailing professional standards.
  • Recommend to the governing authority necessary changes in the policies, procedures, and operations of the agency such that practices remain within requirements and prevailing standards.
  • Disseminate such information necessary and desirable for the operation of the agency.
  • Maintain a sufficient census of adequately trained and experienced team members to care for patients admitted to the agency for care.
  • Develop and implement a patient complaint and grievance procedure.
  • Ensure that all clients are informed of their rights upon admission as required.
  • Obtain contracts with individuals, facilities, agencies, and organizations to ensure safe and consistent care to clients.
  • Provide home health care service only with a written contract providing for the duties and responsibilities of each party as well as those requirements described in regulation.
  • Ensure that all contracts entered by the agency meet all federal, state, and local rules, regulations, statues, and prevailing professional standards.
  • Develop and maintain a program of review and revision of policies and procedures annually or as necessary.
  • Ensure access to the Health Provider network by a sufficient number of properly trained, authorized, and oriented team members to contact the HPN daily during business hours and assure access by the HPN network manager to essential agency team member and agency information.
  • Ensure the development and continuous updating of the agency disaster and emergency preparedness plan so that essential needs for clients are met and the agency cooperates with other providers in serving the needs of the community.
  • Ensure that all required records and reports are kept and submitted as required.
