About The Position

The Chesterfield-Colonial Heights Department of Social Services is hiring a Family Services Specialist to work with adults needing long-term care. This specialist will be responsible for conducting two key types of assessments: Long-Term Care Screenings (Uniform Assessment Instrument – UAI) and Assisted Living Facility Auxiliary Grant assessments. This position involves evaluating adults to determine their eligibility and need for various long-term care services, including placement in assisted living facilities and access to financial assistance. As the Family Services Specialist, you will play a crucial role in connecting individuals with the resources they need. This is an intense and stressful job with critical outcomes, requiring flexibility and prioritization of daily activities. The role involves creating positive change and having a direct impact on the life of a vulnerable adult and their family that lasts a lifetime.

Requirements

  • Minimum of a bachelor’s degree in a Human Services field or minimum of a bachelor's degree in any field with a minimum of two years of appropriate and related experience in a Human Services area (Section 22VAC40-670-20 of the Administrative Code of Virginia).
  • Experience in assigned program area and completion of required training programs or equivalent combination of training and experience which provides the required knowledge, skills and abilities.
  • Current valid driver’s license and good driving record (record must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months).
  • Out of state driving records must be obtained by applicant and presented at time of interview. Records must reflect at least three years of history and be dated within thirty days of interview date.
  • Pre-employment drug testing.

Nice To Haves

  • Possession of a BSW or MSW degree and a Commonwealth of Virginia Social Worker license.
  • Adult welfare experience in Virginia public social services.
  • Experience compiling reports, correspondence, data and narrative of case management and grant proposals.

Responsibilities

  • Handle screenings and assessments that will be entered in the case management systems, Dynamics, Peer Place and Pre-admission Screening.
  • Provide care coordination and assistance to connect customers to services while working with internal and external community partners.
  • Determine appropriate services and write/implement service plans.
  • Provide case management services to monitor compliance for Companion Services.
  • Provide case management for guardianship caseload.
  • Communicate with services providers and many others during the day.
  • Ensure that the case management systems, Dynamics, and Peer Place are updated appropriately.
  • Provide resource information to individuals who contact social services regarding concerns related to adults (phone or walk-in).
  • Ensure coverage for the phone and walk-ins during business hours.
  • Educate the staff and the community about APS intake and mandated reporting requirements.
  • Provide support to the agency in other ways as required.
  • Perform other work as required.
  • Report to shelter duty as part of an Emergency Shelter Team during times of natural disaster.
