Requirements

  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program. Master’s degree from an accredited Physician Assistant program.
  • Current certification as a Family Nurse Practitioner (FNP-BC or FNP-C) or a Physicians Assistant (PA).
  • Active and unrestricted Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) licensure in the state of Tennessee, or eligibility for licensure. Or an Active and unrestricted Current PA license in the State of Tennessee.
  • Minimum of 2 years of experience in primary care preferred; highly qualified new graduates will be considered.
  • Strong clinical assessment, diagnostic, and patient management skills; proficiency with EHR systems.
  • Commitment to continuing education, professional growth, and maintaining clinical competency.

Responsibilities

  • Conduct comprehensive patient evaluations, including medical histories, physical examinations, and ordering/interpreting diagnostic tests within scope of practice.
  • Diagnose and manage acute and chronic medical conditions under established clinical protocols and collaborative practice agreements.
  • Develop and implement individualized treatment and care plans based on patient needs, medical history, and diagnostic findings.
  • Provide preventive care services, including routine health assessments, immunizations, screenings, and health counseling.
  • Manage chronic conditions such as diabetes, hypertension, asthma, and other common primary care conditions with ongoing monitoring and follow-up.
  • Address acute medical issues, including infections, minor injuries, and urgent care needs.
  • Educate patients and families on disease prevention, medication management, lifestyle modifications, and overall health maintenance.
  • Collaborate closely with physicians, specialists, nurses, and interdisciplinary team members to ensure coordinated, patient-centered care.
  • Communicate effectively with patients and families regarding diagnoses, treatment options, and care plans.
  • Participate in care coordination, referrals, and case management to support continuity of care.
  • Maintain accurate, timely, and compliant medical documentation using electronic health record (EHR) systems.
  • Adhere to hospital policies, clinical guidelines, and regulatory requirements.
  • Participate in quality improvement initiatives, performance improvement activities, and clinical audits.
  • Stay current with evidence-based practice, clinical guidelines, and professional standards in primary care.
