About The Position

As a key member of our team, you will work autonomously and alongside other healthcare professionals, utilizing a whole-person approach that seamlessly blends primary care, behavioral health, addiction medicine, and wellness strategies. What You Will Do: Comprehensive Care Management: Develop treatment plans for acute and chronic conditions, focusing on holistic patient outcomes. Preventive Health Education: Guide patients on disease prevention and healthy lifestyle choices to foster long-term wellness. Lifecycle Health Promotion: Adapt health strategies to accommodate the aging process. Diagnostic and Evaluation Expertise: Perform thorough exams and evaluations to diagnose and monitor health concerns. Prescriptive Authority: Utilize your expertise to prescribe medications effectively and safely. Leadership and Independence: Lead healthcare initiatives and manage patient care with autonomy. Denova Collaborative Health LLC is an integrated primary care and behavioral health practice based in the Greater Phoenix metropolitan area. Our comprehensive virtual care services are available for residents throughout the entire state of Arizona. We provide a “whole person” approach to health and promote collaboration among our team of primary care providers and specialists. Requirements
- Master’s degree in nursing.
- Current Arizona Nurse Practitioner licensure
- Board certified Family Nurse Practitioner, with current DEA and ANCC certifications.
- Experience using electronic medical records.
- Current BLS and a valid Arizona Fingerprint Clearance card.
- 1–3 years of Family Practice, Primary Care, or related Nurse Practitioner experience.
- Strong clinical assessment, communication, and time management skills.
- Ability to work independently while also collaborating effectively within a multidisciplinary team.
