About The Position

We are seeking a full-time Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join a busy FM clinic with South Texas Health System Clinics in Weslaco, Texas. Requirements

Licensed Nurse Practitioner or Physician Assistant with Board Certification in Family Medicine
Active or eligible Texas license
Prior family medicine experience preferred
Spanish fluency strongly preferred

Responsibilities

Provide comprehensive family medicine care in person and via telemedicine
Support physicians with value-based care initiatives and HEDIS measure completion
Manage patient transitions from emergency room and inpatient discharge to outpatient follow-up within 7 days
Educate patients on proper utilization of emergency and urgent care services
Collaborate with physicians to ensure accurate coding, risk adjustment, and quality documentation
Participate in after-hours and weekend call rotation
Cover for physicians during leave periods
Champion coordinated care models for Medicare and Medicaid populations
