Under the general supervision of the Family Caregiver Support Program Director, the Family Caregiver Support Liaison assists caregivers to gain information, support and access to services. This includes identifying how to expand the reach and improve programming for caregiver training and engagement. The Family Caregiver Support Liaison is responsible for assisting with coordination, maintaining fidelity and teaching evidence-based programming. Must have the following abilities to perform the responsibilities: Ability to effectively communicate and build rapport with the general public, in person, by phone, and via email, Ability to travel to community and consumer locations within our four county service area (Ashtabula, Mahoning, Trumbull and Columbiana counties), Ability to occasionally travel outside of our four-county service area including but not limited to Columbus, Ohio, Ability to lift, carry, push and/or pull 20 pounds upstairs as needed, Ability to comprehend written material related to essential job functions, Ability to accurately document facts related to essential job functions, Ability to ambulate enough to access locations which may not be readily accessible. Ability to comprehend, recall and apply facts related to essential job functions, and Ability to analyze, evaluate, and implements a reasonable course of action based on available information.
