About The Position

The Program Manager, Fair Table Programs directs, coordinates, and provides oversight for the design, implementation, and continuous improvement of Fairview's Fair Table food is medicine (FiM) programs. This role leads and supports cross-functional initiatives that integrate food security, nutrition support, and community partnerships into clinical and community settings across the Fairview system. The Program Manager serves as a key operational and programmatic lead—translating Fair Table strategy into scalable, compliant, and equitable program delivery. The role partners closely with food resource navigators, clinical teams, community organizations, evaluation partners, and internal stakeholders to ensure consistent implementation, strong patient experience, and accurate data collection that supports sustainability, reporting, and system learning. This position works collaboratively within Community Advancement and across Fairview departments to advance health equity, strengthen community and care site partnerships, and embed Food is Medicine as a core component of care delivery.

Requirements

  • B.S./B.A. in Public Health, Health Administration, Social Work, Community Health, Nutrition, Business Administration, or a related field.
  • 3 years Program management, healthcare, public health, or community-based initiatives.
  • Experience working with cross-functional teams and multiple stakeholders.
  • Strong written and verbal communication skills.
  • Demonstrated ability to manage complex projects and meet deadlines.
  • Experience using data to inform decision-making and program improvement.

Nice To Haves

  • M.A./M.S. in Public Health (MPH), Health Administration, Social Work, Nutrition, or related field.
  • Experience working with Food is Medicine, food access, nutrition programs, or social drivers of health.
  • Experience in healthcare or health system environments.
  • Experience working with grant-funded programs and reporting requirements.
  • Familiarity with clinical workflows, EHR documentation, or patient navigation models.
  • Experience supporting community partnerships and coalition-based work.
  • Project management, quality improvement, or change management certification (PMP, Lean, Six Sigma, Prosci, etc.).

Responsibilities

  • Directs the planning, implementation, and evaluation of Fair Table food is medicine programs across clinical and community sites.
  • Leads or supports program expansion, onboarding of new sites, and integration of Fair Table services into existing workflows.
  • Acts as a primary liaison for assigned clinical sites, supporting referral workflows, documentation practices, and ongoing site needs.
  • Serves as a key point of contact for internal and external partners, including community organizations, coalitions, collaboratives, and workgroups.
  • Identifies gaps, trends, and improvement opportunities and communicates insights to Food is Medicine leadership.
  • Leads cross-functional project teams and workgroups related to Fair Table initiatives.
  • Ensures Fair Table programs are communicated with dignity, cultural humility, and alignment with organizational values.
