JOB SUMMARY Work involves providing for the assessment, care, treatment, and general welfare of patients receiving services on the Forensic Assertive Community Treatment (FACT) Team. May train others as well as delegate to Licensed Vocational Nurse (LVN). Works under moderate supervision, with limited latitude for the use of initiative and independent judgment. DUTIES & ESSENTIAL JOB FUNCTIONS Under the supervision of the Director of Nursing, provides professional nursing services to FACT Team clients within the limits of Professional Licensure and provide assistance to other medical related services within the organization as needed. Responsible for performing assessments on all clients, help keep assessments updated, developing nursing care plans, providing interventions, and evaluating provision of nursing care. The nurse will be an integral part of helping coordinate medical health needs of clients. Frequent travel within the region is required, including home visits and community-based services. The nurse will participate in weekly meetings with team.
