Responsibilities

Conducting initial and ongoing client assessments and interpreting needs into actionable care plans.
Communicating regularly with caregivers to ensure service delivery is aligned with care plans.
Reporting, documenting, and maintaining accurate client files and compliance reports.
Serving as a main contact for clients and families for questions, issues, or care updates.
