About The Position

San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Job Summary: Under the supervision of the Enrollment Manager, obtains initial information relating to admissions, coordinates intake process, and provides information and referrals to community agencies as appropriate. Promotes the St. Paul’s PACE Program to potential participants and community agencies.

Requirements

  • Education: A Bachelor's Degree in business, or a health or human services field, or a high school diploma and office assistant training (preferably health care specific)
  • Experience: Two years of experience in a marketing and/or admissions role for a health care provider, or in an information and referral resource role within the senior network of San Diego. Shall have either training or related experience in the job assigned. A minimum of one years experience in working with the frail or elderly population.
  • Skills and Knowledge: Ability to effectively present PACE philosophy and model to the community and to prospective participants.
  • Working knowledge of physical, psychosocial, behavioral, and family needs of the elderly.
  • Working knowledge of San Diego community health and social service delivery systems, and aging network.
  • Effective written and verbal communication skills.
  • Working knowledge of Medicaid system.
  • Able to assess potential participant’s financial status.
  • General knowledge of financing mechanisms such as Medi-cal, Medicare, Prospective Payment Systems and HMO Managed Care providing risk-based capitated systems.
  • Good working knowledge of PACE philosophy and eligibility/denial criteria.
  • Computer proficiency in word processing, data entry and spreadsheet applications.
  • YOU MUST BE ABLE TO CLEAR FINGERPRINTS THROUGH DOJ AND FBI IN ADDITION TO A DRUG SCREEN CLEARANCE.
