About The Position

The Enrollment Specialist is responsible for the initial screening and coordination of new participants to the Gary and Mary West PACE (GMWP) program. The Enrollment Specialist will partner with the Community Outreach Manager to implement the program's intake, enrollment, outreach and marketing activities.

Requirements

  • High school diploma or GED equivalent required.
  • Current driver license.
  • Proof of auto insurance.
  • Clean driving record.
  • A minimum of one year working in community health, senior health, marketing or enrollment for public health, social service organization or health plan.
  • One year of Medi-Cal, Medi-Care, managed healthcare eligibility or senior related sales.
  • Must take and pass the State of California DHCS PACE and Medi-Cal Marketing Certification exam within 60daysof hire.
  • Must be able to communicate effectively and comfortably in public forums.
  • Prepare concise and clear reports on marketing and outreach.
  • Intermediate experience working with Microsoft Office applications (Word, Excel, PowerPoint, Outlook).
  • Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Job offers are contingent upon a successful pre-employment drug screen, background check, and physical assessment.

Nice To Haves

  • Bachelor's Degree in health or human services, preferred.
  • Bilingual(English/Spanish)highly desired.
  • Experience working with the frail elderly population a plus.
  • Experience working with Electronic Medical Record (EMR) systems (Cognify a plus).
  • Experience working with Customer Relationship Management software, i,e. Salesforce, a plus.

Responsibilities

  • Coordinates with potential participants and/or families to conduct initial screenings and assessments in the homes of potential participants.
  • Conducts PACE program eligibility screening, intake, educational and enrollment assistance activities to initially determine participant needs and eligibility for enrollment.
  • Completes appropriate documentation related to participant assessment as required.
  • Coordinates communications of eligibility decisions, approvals or disapprovals with participant or their family members.
  • Participates in team meetings and activities as needed.
  • Participates in outreach and marketing activities in conjunction with the Community Outreach Manager.
  • Provides input on internal and external outreach, education and enrollment plan for engaging, assisting, enrolling and retaining new and current participants.
  • Facilitates GMWP referrals and community resources for assistance with other needs and complex issues.
  • Provides responsive, high quality service to participants, participant family members, County representatives, outside agencies, members of the public by providing accurate, complete and up-to-date information in a courteous, efficient and timely manner.
  • Conducts public outreach and educational activities, individually and in a group, to participants and community members at clinic locations and other community locations about PACE and other related health coverage options.
