Respite is committed to helping people maintain their independence in the community – to live a life that isn’t defined by their mental illness. We meet individuals where they are at; in settings and spaces where they feel safe and comfortable. The program is a short-term group living environment that provides treatment of intermittent crisis and stabilization of adults with are diagnosed with a mental health/substance use disorder. The goal of the program is to work collaboratively with individuals in developing skills that allow them to live independently and successfully in the community. The Emergency Mobile Respite Navigator provides case management support to adults in the community with complex mental health needs. Many of these individuals need assistance connecting to community resources such as housing, finances, employment, transportation, community providers, etc. Referrals can come from either DMH or the local hospitals/crisis centers. The Navigator will act as the primary point of contact for both inpatient and community providers. They will conduct outreach, engagement, assessment, and person-centered treatment planning for individuals who are enrolled in Mobile Respite. The Navigator is also responsible for managing billing and care coordination with DMH funders.
