About The Position

The ElderCare Solutions and Caregiver Support Manager enhances the quality of life of older adults and their family caregivers by managing various caregiver support initiatives, engaging partner agencies, and overseeing grant-funded projects. Additionally, the program manager provides resources and family consultations to support family caregivers, reduce caregiver burn-out, and engage the person and their care partner in long-term planning for the future by utilizing a variety of best practices and evidence-based interventions. This role also involves supervising staff members who provide intensive geriatric care management services to individuals and their families to maximize functioning and improve their quality of life.

Requirements

  • Minimum of 5 years’ experience in human service, care management, caregiver support and/or dementia/Alzheimer’s disease programming.
  • Master’s in Social Work required.
  • LMSW preferred.
  • Technology – Entering data into MS Excel spreadsheets, MS Word documents and navigating various internet databases, searches, webpages.
  • Listening – Receive information from clients in a compassionate and non-assuming way.
  • Collegially –Being helpful to not only clients but coworkers by providing support and respect and constantly share pertinent updated information.
  • Person Centered – Use motivational interviewing techniques to fully understand the specific needs and situation of each client.
  • Privacy & Compliance – Stay abreast of all compliance and privacy guidelines provided by regulatory authorities through training and following internal policy.
  • Attention to Detail – Following specific guidelines within public systems to complete complicated application processes without errors.
  • Problem Solving – Use guides, help lines and general technical/personal reasoning to navigate applications and databases.
  • Multitasking –Move from project to project, task to task with ease and the ability to return to your original task at a later time.

Nice To Haves

  • Three year formal or informal supervisory experience preferred.
  • Great customer service skills are required and marketing experience is a plus.
  • Health coaching training preferred.
  • Health coaching certification preferred.

Responsibilities

  • Manages a variety of caregiver support initiatives and grant-funded projects, planning and implementing programs, establishing relationships with partner agencies, providing training and support to other JFS and partner agency staff, collecting data, tracking expenditures.
  • Coordinates family caregiver support consultations.
  • Promotes the program internally to agency program staff and their existing clients, as well as externally, by marketing the services to community stakeholders and agency partners.
  • Maintains high quality staff performance by coaching, counseling, training, supporting, disciplining, monitoring and appraising job results for assigned geriatric care management staff.
  • Provides support with agency electronic health record.
  • Applies knowledge of human behavior, attitudes and motivation to the provision of services for client families.
  • Supervises ElderCare Solutions of Michigan (ECSM) staff members who provide intensive 24/7 geriatric care management services and complete administrative tasks as assigned.
  • Facilitates and coordinates the family caregiver support program and utilizes evidence-based and best practice interventions, including the SHARE Dementia program, designed to provide coordinated supports, resources, and services to assist in the reduction of caregiver stress, with the goal of improved quality of life for both caregiver and older adult.
  • Provides geriatric care management services for ECSM clients as needed.
  • Coordinates and run various culturally-sensitive support groups for family caregivers and education workshops in partnership with other internal and external stakeholders.
  • Utilizing a person-centered model for planning, assesses family caregivers for assistance; identifies appropriate interventions; identifies persons recently diagnosed with early- stage dementia and their care partners to assess functional ability to benefit from participation in the SHARE Dementia planning program.
  • Evaluates the need of the caregiver for supportive resources and services that will enhance the quality of care and effectively support the caregiver and the older adult.
  • Conducts comprehensive psychosocial assessments for ECSM as required.
  • Monitors and matches individual client needs with available services and resources.
  • Provides assistance and intervention in implementing and revising the person centered care plans to meet client needs as changes in status occur or at review time.
  • Assists Human Resources and applicable management staff in recruiting, orienting, evaluating and training staff members.
  • Plans and facilitates community-based outreach activities to provide education on the value of caregiver support and utilization of evidence-based practices, including the SHARE planning program for persons diagnosed with early-stage dementia and their care partner.
  • Provides community training utilizing best practices and evidence-based interventions, including Powerful Tools for Caregivers and others.
  • Assists the program director in carrying out a marketing plan for older adult services.
  • Demonstrates awareness of the importance of accountability and fiscal integrity by completing all required forms, preparing and completing in a timely manner all required records and reports for program clients; execute grant reporting including pulling reports, analyzing data, and completing reporting documents/letters.
  • Utilizes data from program evaluations to inform agency strategies and decision making.
  • Participates in best practice and evidence-based practice trainings.
  • Complete all trainings, workshops, and shadowing of SHARE licensed professionals required to deliver the SHARE Program.
  • Completes all required follow up trainings and activities necessary to maintain this licensure.
  • Maintains professional and technical knowledge by attending agency in-service programs, educational workshops, reviewing professional publications and establishing personal networks; Shares professional knowledge with staff by seeking out professional development opportunities and sharing new learning with staff.
  • Assists other departments, as needed, in the event of a program or circumstance that requires additional assistance and staffing support.
  • Adheres to agency and client protection policies by regularly acclimating yourself with the JFS privacy, safety and security policies made available within the employee handbook.
  • Participates annually in agency and partner related philanthropic activities and campaigns.
  • Embodies the mission and values of JFS in all work done on behalf of the agency. Utilization of the JFS key strategies will ensure success in this accountability.
  • Other duties as assigned by management.
