The Arc of Opportunity provides choices for individuals with disabilities to live in a home where they will be comfortable, whether in a private apartment, the home of a family member or caregiver, or a group home. Home and Community Based Supports consists of a variety of programs including, but not limited to: Adult Foster Care (AFC), Individual Support Services (ISS), Shared Living (SL), and Money Follow the Person (MFP). Below is a summary of each program listed: 1. AFC: This program is a medicaid-funded initiative that enables individuals with disabilities to live with caring providers in a safe and nurturing environment. We provide comprehensive case management and nursing oversight. 2. ISS: This program supports individuals living independently in the community to maintain and expand their independence and pursue their dreams. We provide medical coordination, financial coaching, community integration, emotional/behavioral supports etc. 3. Shared Living: This program pairs adults with disabilities with non-family members who have the desire, ability, and available accommodations in their home to support an individual as a Home Care Provider. 4. MFP: This program helps individuals with disabilities and seniors who want to move out of nursing homes transition back into living in their communities. MFP enhances individual’s opportunities to live independently and improve their quality of life.
