About The Position

The Discharge Planner is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient’s post-hospitalization needs while ensuring continuity of care. No supervisory function. Note: The essential job functions of this position are not limited to the duties listed above .

Requirements

  • Bachelor's Degree from an accredited college or university in Social Work or a clinical-related mental health field
  • Minimum of one (1) year experience in related disciplines with hospital experience strongly preferred.
  • N/A.
  • CPI or Safety Care training within 90 days of employment and before assisting in a restraining procedure.
  • Must be able to pass a competency exam for the area upon hire.
  • Must have CPR/First Aid (BLS) certification at the time of application or arrangements can be made to attend the course on campus.
  • May be required to work flexible hours and overtime.

Responsibilities

  • Communicate with the referral source within the first 24 hours of admission.
  • With assistance from the Care Center, social workers and nurses identify all outside professionals currently working with the patient so that appropriate discharge plans can be completed and appropriate communication patterns can be facilitated to minimize duplication.
  • Identifies needs of referral sources and/or professionals during hospitalization.
  • Oversees the tasks of continued communication of treatment progress, discharge plans, and discharge time to outside interested parties and facilitates the transfer of required documents to the outside agents.
  • Interact with the Business Development Department to assist with determining customer satisfaction and problem identification and finding solutions.
  • Responsible for discharge plans, including scheduling appointments and communicating with parents and significant family contacts.
  • Assists with chart audits and completion of medical records.
  • Inform patients and families on discharge planning options based on diagnoses, prognoses, resources, and preferences related to home care services.
