About The Position
Discharge Coordinator Full-Time The Discharge Coordinator will work with parents, outpatient providers, and therapists to establish d safe and supportive aftercare plans. Requirements
- Bachelor's Degree required
- 1 year experience in Case Management preferred
- Experience and/or knowledge of mental health systems
- Organizational skills
- computer skills including excel
- strong communication skills
